$1,500 PPO DEDUCTIBLE PLAN
This plan has copays for things like Physician Office Visits, Urgent Care, and prescription drug coverage without having to pay the deductible first. The deductibles are $1,500 individual/$3,000 family. Once you reach your annual deductible, your qualified expenses are covered at 80% up to the out-of-pocket maximum of $3,000 individual/$6,000 family. This plan is eligible for a FSA.
$4,000 HDHP DEDUCTIBLE PLAN
Once you reach your annual deductible, your qualified healthcare expenses are covered at 90% until you reach your out-of-pocket maximum. Those who take full advantage of a Health Savings Account (HSA) find the low monthly premium to be an incentive to choosing this plan. While there are annual contribution limits, HSAs allow you to roll over your balance from year to year. And your contributions to an HSA are pre-tax, which is a big advantage.

A TPA is an entity (such as Auxiant) contracted to establish and provide administration for a health plan, such as nbkc bank. A TPA is not an insurance company.
